Zac Afeaki

Zac has extensive senior HSE experience across oil and gas and other high-risk industries, including a long tenure at a major LNG Major Hazard Facility. He currently provides ongoing senior HSE support across complex, high-risk industrial and major-project activities.

His work spans safety-in-design input with project engineers on shutdown and downstream facility project scope, critical-risk assessments (risk registers, CRAW, HAZID), and governing the incident reporting and investigation process for a national business unit, including HSE campaigns developed in response to incident trends.

At a major oil and gas operator, Zac introduced Human and Organisational Performance (HOP) and High Reliability Organisation (HRO) principles into business unit leadership and safety conversations. He developed and implemented a 5-year Learning Organisation Roadmap, managed the roll-out of the 'Our HSE Behaviours' and 'Just Culture' frameworks, initiated Usability Mapping to make key procedures easier to read and use, hosted an annual HSE Contractor Collaboration Forum and owned the business unit's Psychosocial Management Plan.

Zac moved into HSE from another service career, landing his first HSE advisor role and never looking back. He's a committed lifelong learner, and over many years in high-risk work he's picked up lessons you can't get from a textbook. He started Anqor to tackle the gaps and opportunities he kept seeing across industry, and to help organisations solve their HSE problems thoroughly and quickly.
